You will have your own personal Health Advocate in Doctor Simong Youmans.  Take back the responsibility and control over your own health and make the decisions regarding your health care for yourself. Doctor Youmans goal is to keep you healthy as opposed to most Doctors who only see you when you when you are ill.

COST:                       $2500.00 annual membership         

Individualized Health Maintenance Program

Total Cost of Elite Wellness Club Membership is $2,500 per year

A deposit of $1,000.00 is required to be paid in full before any services can be offered.  We do have options below for payment on your membership. All monthly payments will be taken out on an automatic payment setup of either credit card of check (EFT). 

·         One time payment in full $2,500.00

·         Deposit $1,000.00 one time in full, and monthly payments of $150.00 credit card/check(EFT) over 10 months

·         Deposit $1,000.00 split in 2 payments of $500.00 in first 2 months followed by monthly payments of $150.00 credit card/check(EFT) over 10 months

 MEMBERSHIP INCLUDES:  

·         1 Comprehensive Physical Exam by a board certified physician (personal and confidential) = $375 value

·         2 Office visits used anytime during that year + $200 value

·         Comprehensive Laboratory Testing including CBC, Electrolytes, liver function test, and lipid panel =$400 value

·         Additional Screening based on risk factors and age :  EKG, Pulmonary Function Test, Chest X-Ray =$225 value

·         The balance of your account can be used towards a variety of other lab tests, imaging studies and office visits available at pre-negotiated cash discount prices.  Special vitamin testing, allergy tests, neurotransmitting tests and extensive metabolic testing with weight loss program available on site.

·         Personalized intensive counseling for anxiety, weight loss, and stress management available.

·         Personal fitness trainer available

·         Reike, energy healing, and therapeutic massages available

·         No need for cash.  You will be given a membership gift/credit card which can be used in exchange for services rendered.  The amount will be deducted from the annual fee.

·         10% deduction on beauty treatments from the spa side, including facial, botox, fillers, laser treatments, and manicure/pedicure. Skincare products, supplements and other products are also sold at a 10% off discount to members.

·         Expedited referral to specialists as needed by Dr. Youmans

·         Concierge service: One call is all you need to make to a patient liason and she will make all arrangements for your medical needs and appointments.

·         No copay.

·         No pre-existing condition.

·         No pre-approval for testing needed
